Problem:
While testing the stack they closed the hydraulic crown valve so they could test the gate and the grease fitting blew out of the valve and went about 30 feet. Blowing grease everywhere.

Troubleshooting:
Reason:
We suspect that the grease fitting was left partially loose on the valve. When they closed the valve with pressure in the bore the pressure blew the grease fitting off.
Solution:
We need to discuss with Shop and Field personnel about the dangers of pressure and make sure that the grease fittings are checked before and after testing in the shop. The grease fittings should also be checked on location when rigging up.
